What is First Link™?

First Link™ is a program that connects individuals and families affected by dementia to opportunities for learning, community services and support.

If you or your family member have concerns about changes in memory, personality or abilities, or have been diagnosed with Alzheimer's disease or a related dementia, then First Link™ is for you.

  • First Link™ gets you connected to support early and will support you throughout the course of the disease;
  • the more you know about Alzheimer's disease or a related dementia, the better you may be able to cope; and,
  • you will be linked to community supports and services.

Who can make a referral to the program?

A referral can be made by anyone (self referral, physician, allied health professional, community care partner, etc.)

How do we make a referral to the First Link™Program?

It’s simple – just fax, phone, or e-mail a referral form and First Link™ will take it from there.
